Parents must complete the Health Services Consent online in the parent portal. You may find instructions on how to complete all online forms here: OCPS Parent Portal Online Forms. It is very important that every student have a completed Emergency Student Contact Form on file with the school.

Parental consent does not apply to emergency services. In the event of an EMERGENCY, I understand that the school will access the 911 emergency medical system.

OCPS Medication Policy

When possible all medication shall be administered outside school hours at home.

For safety and security reasons, students are not permitted to bring medications to school, this includes prescription and over-the-counter medications. Students may carry FDA approved over-the-counter medications for headaches, for their personal use only. Students should never share their medications.

If your student must take medication at school, (including over-the-counter medications, with the exception of headache medications) the following OCPS policy will be followed:

  1. All medication must be administered by the school nurse or designated trained person.
  2. The parent/guardian must bring the medication to school and complete the authorization form. Please email the school nurse to plan for medication drop off.
  3. Over-the-counter medications will only be accepted in the original factory sealed container.
  4. Prescription medications must be in the original pharmacy labeled container. Parents can ask their pharmacist to provide them with two containers: one for school and one for home. Medications including eye drops cannot be transferred to and from school by the student.

Students may carry inhalers, diabetic supplies, pancreatic enzymes, and Epi-pens, provided the proper authorization forms are on file with the school. These forms require a physician's signature and are available from the school nurse. Please send an email request to the school nurse for these forms.

Illness

Please keep your student home if they have a temperature above 100.4, until they are fever-free for 24 hours without the use of fever-reducing medications

Injuries

Please provide the school nurse with medical documentation of any injury that requires restrictions/accommodations ordered by the physician. Medical documentation is required to be excused from HOPE.
